How a HIFU facelift works without surgery

High intensity focused ultrasound crosses the skin without breaking it and deposits heat at precise depths: the SMAS layer around 4.5mm, where a surgical facelift would pull, and the dermis at 3.0mm and 1.5mm. The heat triggers new collagen over the following weeks, which is why a HIFU facelift tightens gradually rather than overnight. What the clinical evidence shows

Two findings are worth knowing before you spend anything on a HIFU facelift. A 2023 systematic review pooling 16 clinical studies (Contini M, et al. International Journal of Environmental Research and Public Health, 2023; n=337 at the 90-day mark) found 92% of patients showed improvement on an investigator aesthetic scale at 90 days, with the benefit concentrated in mild to moderate skin laxity. A prospective trial of 93 patients (Oni G, et al. Aesthetic Surgery Journal, 2014) is the honest tempering of that number: blinded reviewers, who rate more conservatively than patients scoring themselves, saw improvement in 58.1% of cases, while 65.6% of patients perceived improvement in their own skin. The same trial found most patients with a BMI over 30 saw no measurable change, which is one of the specific things the assessment screens for.

A third study, this time in 25 Asian patients (Lu PH, Yang CH, Chang YC. Dermatologic Surgery, 2017), measured a statistically significant lift under the chin at 90 days, with the effect tapering by 180 days. That taper is the clinical basis for reviewing results at six months rather than assuming a single treatment lasts forever. These are population averages from controlled studies, not guarantees; individual results vary with age, skin condition, laxity severity and body composition.

The "melted face" fear, answered

The loudest fear around ultrasound facelifts comes from forum stories of hollow cheeks after treatment. Here is the honest version. The energy target is the SMAS and dermis, not the fat compartments below them; correctly placed shots tighten skin, they do not dissolve fat. The horror stories almost always trace to depth and energy settings that were not calibrated to the treatment area. Correct calibration, not any particular brand of machine, is what determines whether the result is a tightened jawline or a hollowed one. When a non-surgical facelift is not HIFU: RF, biostimulators and fillers

HIFU is not the answer to every sagging face. If laxity is early and mild, monthly RF skin tightening sessions build firmness at a gentler intensity and a lower per-session price.
